You are not authorized to take this survey

I am trying to create my first survey in our Dev instance. I have created the survey and made it accessible to anyone in the Availability tab on the Survey Designer section. I have configured the trigger conditions. I have enabled email sending and at the moment all emails are set to email to a test email account.

The survey invitation email is being received by the test email account. However, when I click on the link I get a message saying "You are not authorized to take this survey". I tried impersonating the test user but this does not work. I then change the email address to my own email address so I was clicking on the link from my own mailbox while logged into Service Now as myself and I still get the same error.

I found the KB0597506 which explains a workaround with replacing some script in from the assessment_take2 UI Page but I am unsure what module to find this in. Can someone please assist with telling me where I can find this?

I am very new to Service Now and finding most things by fumbling through. Any assistance would be much appreciated.

To access UI page, on the left navigation, you will find the UI pages menu under System UI application.

Use the name to find the page you are after.

I think I have found the section of the script that I need to delete and replace. I am not a coder so am a bit nervous to make the changes in case I make a mistake and break it. I think I will sleep on it and give it a crack in the morning.
