Create a UI Action?

Hello,

I created a catalog item and one of the fields is to "Add Access" or "Remove Access".  If "Add Access" is selected 

create a UI ACTION – a field labeled “Add Access for these locations” displays.  The field should Reference the Location table and be multi-select.  First, I created a UI Catalog Policy, please see attachment. Then, I created a catalog UI Policy Action, please see attachment.  However, I do not how to go to select the reference field-location table.  Thanks!

I am not sure I understand your requirement... but what I can tell you is that your Catalog UI Policy Actions will not do anything, because you have there Leave alone for all attributes - read only, visible, mandatory...

If you want to change any behaviour, than update the value from Leave alone to true/false according to your exact requirement that is unclear to me.
