Creating an Incident using create new module error (Unique Key violation)

When filling out an Incident form from the new module and clicking Save, I'm getting an error 

Message
Unique Key violation detected by database ((conn=1351711) Duplicate entry 'b00d652a1bdfca50cd2a5314604bcb22-78e54f3b1bae4610cd2a5314604b...' for key 'task')

Hi @Ashley45 

 

Did you install any plugin recently which has an impact on ITSM modules? If yes, try to repair that, also try to repair the Incident plugin once and check.

Not seeing any recent plugin installs

okay... then try to repair the Incident plugin and also did you check the sys id, where it belong?
