unable to see top banner on service portal page

I am unable to see the top banner in service portal where it says: Knowledge, Catalog, Requests, System Status, Cart, and Tours
What I expect to see:
but what I see instead:
I checked my request filter but everything seems fine. I am thinking it is a ACL list issue?
Any suggestions?

I had to undo the change from here (click see comparison):
then click revert.

Does the issue still exist? If yes, then it’s better to log a new support case if it’s a client instance. If it’s a PDI, close the current incident and create a new one.

I already have SP Header Menu for main menu. The theme SP theme doesn't exist.
