Attempting to run a report to show responses to assessment

I'm trying to run a report to show the responses to an assessment instance.  Everything I've tried is falling short of actually showing the responses to the assessment.  

Any ideas?

Can you share a screenshot of the report filter conditions and the table you have used.

Which table are you trying to utilize through report designer? have you utilized asmt_metric_instance_question?

I'm using asmt_metric_result table using pivot style i displayed the table like that

Column : Metric

Row: Instance.TriggerID (if you wan to based on RITM Number)

If the survey having comment then the comment are not able to display as string in this way, so i do it separate report just to display the comment.

No sure if this what you are looking at.
