This usually happens because Dashboard report widgets and normal list/report views do not always use the same personalized column configuration.

When you added the report to the Dashboard using filters, the dashboard widget may have inherited:

  • Different list layout settings

  • Saved visualization configuration

  • Personalized column preferences

  • Report source configuration

Regarding “Reset to column default”:
When you clicked it, ServiceNow attempted to revert to the system default column layout. If no proper default list layout was configured for that view/table/report, the selected columns can appear blank or removed temporarily.

You can try the following:

  1. Open the report directly (not from dashboard)

  2. Configure the desired columns again

  3. Save the report

  4. Re-add or refresh the dashboard widget

Also check:

  • Whether the dashboard is using a different view

  • Personalized List settings (sys_user_preference)

  • Report visualization type

  • Interactive filters affecting displayed columns

Sometimes clearing personalization and reloading the dashboard also helps.
