How to add a search option in a specific dashboard?

I want to search a specific dashboard I created with these fields

1. 1. short description - Keyword
   2. description - Keyword
   3. category 
   4. sub-category (Primary Issue)
   5. Symptom - Secondary issue

This should work only for a specific dashboard and inside that dashboard, I want to add this search feature

can you please share the code that you have written for the description search on dashboard?  Also, if want to work this code for multiple tables where we need to do the changes.
