How to create report on CSAT Survey in CSM

We have customer service survey and want to create data visualization to have below fields on the list:

Case Number

Subject

Contact

Survey Questions

Survey results for each question

 

How can we achieve it? Which table to use? How to dot walk to have above mentioned fields.

This can be achieved by a database view to combine the different tables in one view.
