How do I link a Incident to a survey

Hello,
When an Incident is resolved an email is sent to the customer to notify them of this.
Within this email I want to add a simple survey just asking how the experience was.
When it is submitted the Incident number should be linked to this survey.
Does anyone know how to do this or any ideas the best way to achieve this?

Thanks - This is helpful but still not answering my question.
How do I add a link to the survey at the end of a resolution email and then have that Incident number show on the results?
