How to remove survey landing page and redirect directly to the survey question page

Hi,

Can someone help me with achieving the following for a Customer survey?

• Removing the survey landing page (attached picture: landing page)
• Redirecting to the survey question page from the link that's sent to the user(attached picture: survey questions)
• Including the incident/request number of the ticket on the top of the survey question page (attached picture: feedback number)
