Mutiple selctions on a form

I would like to be able to select various multiple selections from a pre determined list shown on a form in the servicecatalog
In the example below I would like to be able to select 3 options from the list shown e.g. Docking station, Monitor and mouse a the same time

If i understood, why not you use Order Guide. Please check |OOTB New Hire catalog item in PDI and share feedback.

Hi - thanks for this - I have tried the order guide - the issue now is that one of the options contain a list from which more than one pick is required - so 2 need to be selected from 5 options are needed - how do I make this happen?

So you want , out of 5 , a user must pick minimum 2 ? Any screenshot of same from portal.

any number of selections can be picked from the 5 options

Got the point now, so we need to convert the data type. Currently it is radio button , we need to change to check box.
