I would like to be able to select various multiple selections from a pre determined list shown on a form in the servicecatalog
In the example below I would like to be able to select 3 options from the list shown e.g. Docking station, Monitor and mouse a the same time

When I have impiut field listed below , I get a box saying 'Choose Options' and when submitted via Portal, it seems to just loop

So you have created 22 variables , for the order guide and once once user pick this or fill this and click on Next then he / she will see the relevant catalog items linked with conditions.
This is how order guide work. If required lest connect 1-1 , message me on community to connect.

I started the starter, movers leavers process by creating everything as an item - should I be taking out the functions (starter, leavre, mover) as order guides?
I have completed all the fields on the form and still get the 'Choose Options' button?

If you are using ITSM then I will say, better to create 3 different order guide for starter/ mover/ leaver and use the variables accordingly. Then on based on varaible, show the catalog items to be fill by user.

I tried your problem in my PDI please refer below solution
You can do this in two ways
1. Create List type field
2. Create checkbox type field and in response you will get true or false you can able to select multiple
