SC Task fields like Requested for, Request Item and Item are coming

Hi .

 

SC Task fields like Requested for, Request Item and Item name are coming as blank after submitting the catalog form (This catalog form runs on Sc_Request table) after the form submission it will create a sc task from there these fields are coming as blank.
