service catalog flow is not triggering

i have created a flow and attached to one of my catalog item.
the flow is activated and attached to the item but when i order the item flow is not triggering

the approval record is rejected but still the flow is considering it as not rejected

Did you try testing multiple times?

i have tried with multiple records but still the same.

I haven't encountered this in the past.

is this happening with other catalog items as well which use Flow and have Ask for Approval action?
