Questionnaire - why do I do see a header of Variables ?

Any idea, why on this instance when I show a Questionniare I see Variables after the title and before my question ?
I cannot see why it suddenly started. I did wonder if it was an instance, plugin update, but a PDI that is to the same level is not showing this.

you mentioned a questionnaire, is it related to Assessment (eventually Survey Designer)? Or where is this taken from?

this is the author using an account on a personal laptop vs work one.
It is in a Playbook. I can ask a quesitonnaire on a PDI and I do not see that, but on this client instance it started recently and I have no idea why
nothing on the forms that I can see - just bizarre and annoying

If im not incorrect, this is a bug that was patched and fixed.
"Form section headings were incorrectly displayed when a Playbook activity contained only a single form section. Section headings are now suppressed for single-section activities. For multi-section activities, the first section heading is also correctly hidden while subsequent section headings remain visible."
