Please help me to create report to get monthly open incident count.

You can create report like below.

Select any of score type

Hi @nehasingh98 ,

 

You can navigate to your target dashboard and click on + icon which is there in top right corner and then select Report as widget category. Now look for your report and place it on dashboard.
