Embed survey number scale in Microsoft Outlook

Hi All,

I have requirement to create an outlook embed survey in numeric scale. Please refer the image.

Can anyone help me to create it. I have tried by enabling the outlook actionable plugin and created one survey question. But it is not looking like as I wanted.

Can anyone help me to achieve this functionality.
