Flow Designer populate a field on item B with a variable from item A

I have an order guide setup with sequencing so that item A has to complete before item B is assigned/launches.  I want to grab a variable value from item A to populate a field on item B.  Both items are under the same request so I'm assuming I have to somehow lookup the current request, get the variables from item A and then populate the desired field in item B.  I have tried a few things I have found here on Community, but can't seem to get the exact steps I need to make it work (keep getting no record found in look up record errors).  Could anyone provide me with a step-by-step approach to achieving this?  I have some experience in flow designer, but not a lot.  Thank you for any help.

Attaching screen shots.  It is Step 11 that fails. The screenshot with time of 9.31.12AM is the error I get.

@MonicaW

here give the value of the Namefield

see below

Also Step 12 not required

You get the value directly from variable ownership record once you get Step 11 working correctly
