'Send for Review' pop-up for Service Maps

When requesting a Review of a Service Map, a pop-up form appears with the following fields:

Request Type = Review Request (auto populated),

Assigned to (auto populated with Service/Business owner)

Business Service (auto populated), and

Description.

I would like to add the 'Short Description' field to this form but I am unable to find where the form is configured.

Can someone point me to where this pop-up page comes from?
