I have a requirement of making a service catalog item where there is a form that can be filled in with questions and then once entered the information is carried over into a request ticket. Would anyone be able to advise how this is possible?

that's what the OOB service catalog does
1) you create catalog item with questions/variables
2) end user submits it and then REQ, RITM gets generated and the questions/inputs user gave are visible on RITM form

you can create variables associated to your catalog item
Simply navigate to docs and it has extensive documentation for the same
Create a service catalog variable
