Some context details would be helpful.
- Does the incident task still get submitted/saved?
- Are there any reference filters/qualifiers applied?
- Is that the oob incident task table? (Our org uses a custom one with the same name)
- Has this always been an issue or did it start happening recently?
- Is there another incident with the same number?
