Hi @supriyapriy ,

 

I am not sure what your thought or what you want to achieve, but if you want to create or if you want to add a new field to the Incident form, follow these steps:

 

Right-click the form header and select Configure → Form Layout, or use Form Designer for drag-and-drop feature…In Form Layout, select fields from the Available list and move them to Selected, then adjust their order. You can also create a new field on the fly (e.g., a new Choice field) and add it directly.
