approval gets triggered

Hi ,
The approval activity in the workflow gets triggered. It is placed after the approval user activity. What could be the possible reason for this ? I tried searching for business rule on the RITM and Sc task table but couldn't find any.

Need to check in flow is any thing mentioned after X , Y should go.
