How to create Catlog Items

Catalogs create with category, catalog, with showing service portal, groups, variables and variables sets, fields etc.

i want to create one form and in that form all fields

If you want to create catalog item, category, add catalog item into categoty, catalog item will be visible in Service portal, please follow the below steps :

Hi @radhakawade

To create a catalog item in ServiceNow, you can use the standard platform form by navigating to Service Catalog > Catalog Definitions > Maintain Items and clicking New.
