Hi Community,
I have a requirement in ServiceNow where we need to capture a Design Intake Assessment against a record in the Scrum Task (rm_scrum_task) table.
We are looking for an OOB approach, preferably using the existing Assessment framework, rather than creating a completely custom questionnaire/table.
Requirement
On the rm_scrum_task form, we need to provide a Related Link such as "Design Intake Assessment".
When the user clicks the link:
- A set of Design Intake questions should be displayed.
- The questions should be completed and submitted by the user.
- The submitted assessment should be associated with the originating rm_scrum_task record.
- The completed assessment/response should then be visible under a Related List on the Scrum Task.
- We need to retain the submitted responses so that they can be viewed later.
The assessment contains a combination of:
- Text fields
- Email fields
- Choice questions
- Multiple-selection questions
- Long-text questions
- Date fields
- URL fields
- Attachment requirements
The preferred solution is to avoid creating a custom assessment table if possible and leverage the existing ServiceNow Assessment functionality while ensuring that the submitted Design Intake data remains traceable to the rm_scrum_task.
Any guidance on the recommended OOB architecture/configuration would be appreciated.

A good OOB approach would be to link the assessment instance back to the originating Scrum Task and expose the completed response through a related list. You should check the existing Assessment framework and reference handling first before building any custom tables.

Hello,
Try to use the OOTB Assessment/Survey framework and relate the assessment instance back to the Scrum Task.
Assessment Framework already stores the responses in tables such as Asessment Instance, Metric Result, etc.
Hence, you may try to utilise the Survey Model, if customisation is not the option.
Later, place a UI Action Link in the Related Links section,
which displays the submitted result.
