Upgrade Test Plans. Template?

We're looking to upgrade from Calgary to Eureka and need to develop a test plan. Does anyone have a generic test plan they can point me to so we don't have to reinvent this particular wheel?

Is there any links that work now?  that have upgrade test plans? Since our team is new to software testing, they don't know how to test or what to test.
