Upgrade Test Plans. Template?

We're looking to upgrade from Calgary to Eureka and need to develop a test plan. Does anyone have a generic test plan they can point me to so we don't have to reinvent this particular wheel?

I good way to start to create the test plan is to use the List of requeriment or the acceptance criteria.

So the team can focus the test what is important to bring value.
