While going through the upgrade skips, I reverted one of the skips to base system - I want to undo this. I went back to the record and changed the resolution to Reviewed and Retained and clicked Apply Changes, but this did not undo it. The change is on our Press Release form, it added a bunch of new related lists that we don't want, and removed our customized ones that we need. How can I undo this and add it to our update set?

You can go to the form and add the Version History Related List - ( like the one you see on a Business rule at the bottom) - then see if you can find the old version and replace it - i think this would be if the changes also changed the Form view aside from the related lists.
OR if you have the old view you want in another instance just go by side by side and manually fix it -- if all it is is removing some extra related lists and nothing else right click and select configure related lists and remove.
There may be another way and ill be interested to see it.

There's no version history list, there's also no way I could go back and add the original related lists because they were customized.

Just to clarify - after doing a revert to base system you would go back to the skipped record and then click Reapply Changes to undo it. If that didn't work than you could go to the record directly and revert it to the previous version or re-add the custom related lists if I'm understanding correctly
