Update Sets and Scoped Applications not tracking all updates

Seems that since Washington DC - Patch 7, some changes and records are not being properly tracked in Update Sets or Scoped Applications.

1. Change to a script on an Inbound Action in Global missed last change.

2. Connection record not being tracked in Scoped Application, shows it is in sys_scope of the application but when I created a new update set and used the Add to Update Set action it created a Global scoped child to put it in.  

 

This is causing delivery from lower to higher environments to fail as things are missing that are necessary.

 

Anyone else bump into this?
