Client side behavior on Assessment / survey

I have a Multiple selection question on a Questionnaire, an option being 'Not Applicable'.

I want to configure that if a user selects 'Not Applicable' then the user should not be able to select other options.

Like here, If I selected NA, then I should not be able to select Option 1 & Option 2.

 

I heard we can make changes in 'assessment_take2' UI Page, but not sure how that can be done.
