Create Incident topic block not working

Hello Team,
I duplicated the out-of-the-box Create Incident topic, and removed Universal Request from it, and used the same topic block in the Generate Incident topic. However, when I checked in the service portal, it does not display the incident number or hyperlink for that incident.
Can you please check the below screenshots and suggest any missing parts?
