HELP!!! Bot Response Card - giving a summary of the conversation

I really would appreciate some assistance with this... I've been looking everywhere and don't know how I can get the bot response to give a summary of the chat conversation. Can someone please show me on how I can create this???

 

I would like the yellow conversation to be summarized in the Card bot response. Is that possible? Thank you in advance!!

@LearnnTravel  Please find my observations below for reference, 

 

In the OOB, we have a topic block called "Survey" which generates the "Response Summary" as below,

In your scenario, please check if there is any custom topic blocks created with specific questionnaire (or) filter it with "Topic Blocks" and review it. We faced the similar issue for "Agent Chat" configurations and found the questionnaire where from this above topic and the questions were in assessment table.

@LearnnTravel  From the above screenshot, I have configured the pre-chat surveys that needs to be triggered on initiation of the chat. Your required to configure the assessment questionnaire on the Assessment metric table for pre-chat surveys.

Thanks, helpful to see your screenshot and that's what I'm looking for but it's a bit too complex for me. Could you help me break it down further (if possible)? I checked the survey topic out but can't seem to find out where can I start to make mine simple. Any other suggestions or videos I can watch? Thanks.
