I have configured the parameter screen for Reassign sc task for Now Agent mobile and not working

Hi
Currently I am configuring now Agent mobile app for our Dev instance, as part of configuration I am working on the Re assign button/screen.
I have configured Function, function instance, action item and parameter screen similar to Incident which is already comes OOTB. This issue is on the parameter screen configuration. I have configured everything and still it throws error and page is not loading.
Need help, where did I went wrong?
But no issue with Incident

Need to add the attributes for the inputs provided. Here the inputs are assignment group and assigned to and attributes are like source table, source field name and target table.
