need to trigger survey notification when incident state is closure

when incident is closure survey is not triggering even though probabliti is set as 100%  which plugin we need to activate  and what are steps we need to follow please help me with this query!

Survey is an OOB component, no additional activation should be necessary.

Please add some additional information and, if possible, provide screen shots on your current setup to enable the community to help you.

I had verified email logs once incident was closed survey is not triggered only closure information I am able to see, please find the attached screenshot
