Record Producer || Unable to create record when supplied with information

Table Name - Gym Membership 

Record Producer - Enrollment in Gym 

Below is the snip of Record Producer - 

When I enter all the values in Record producer. Instead of creating a new record it takes me 

to below page asking for same details again. 

Here , when i enter the values and save it -> Then I am able to create a record. 

I want to create record in the very first go.

The script in the record producer. What does it do? Does it have a redirect, or something like that?
