When I tried to reassigned a new group to existing incident record several times, Reassignment count field is updated according to assignment, but it is visible mismatch count on activity stream of an incident.
Anyone has any idea on this why activity stream is not covering all the assignment group. It's showing updated 4 record field on activity stream but not all
on activity stream it showing as only 1 times

Incidents include a field called 'Reassignment count' which records how many times an Incident is reassigned. However this only counts when the assignment group changes, if you reassign between users in the same group, the value will not update.
to check all group information in Activity which are changed
just click on filter activity icon
scroll down and click on Configure Available fields
it will bring this page
search Assignment Group field add to selected
then click save
after that you can see all group changes information and as below
from this you can recheck count
