Make sure you follow below steps below correctly:

1) Table = Incident and Form context menu is true.

2)Check the condition of the UI Action.

Recently we  have upgraded to Washington version. so it is not showing.  is there any replace for the "Show live feed". ui action in Washington version.

Yes, Even for me that UI Action is not working. I'm not sure how that button is useful. Let me check if I find any document I'll share with you.
