Does a user need Unrestricted user license in APM (EA) to fill and assessment survey assigned to them?

No. Assessments can be shown on the portal to end users, so no role required.

Having said that: anything to do with licenses can only be trusted when it comes from your ServiceNow sales rep. Nobody on the community knows your license agreement with ServiceNow.
