Fallback Topic is not working

Hi Guys,

 

It's been almost 3 years since we implemented VA. Since then we have not made any major changes. Looks like things are starting to break. For example, the Fallback Topic is not working. Users are getting this message :

I found some articles in the community but nothing helps.

Could someone please help? We now have a lot of incidents created because of this.
