How to add stage for approval in flow design instead of action can someone help me with this below is the flow screenshot

Not 100% sure what you mean, but 'stage' is just a field on the RITM, so you can just set the stage on the record. 
You can also set stages within the flow itself: go to the 3 dots on the top right and select 'flow stages'.
