Duplicate text in incident record after conversation with live agent in ServiceNow

Hi Everyone
In ServiceNow Plateforme there is a live chat feature.
While user connected with IT service Desk user in live chat .
If ask any query like "Hi, how we can help you "this text duplicated in two times
It has happened only in additional comments of Incident form .
Anyone who can suggest , how it can be resolved .
I have checked folowing options-portal configuration 
checked sys_cs_conversation.list table 
widget etc. Also sercahed for solution in all places but still not getting any solution.
All activities where it is stored , how all conversation activities stored in the incident
