Recipient List built on Solution ID field

Good day

I am looking to create a recipient list based on unique form on below condition builder is like so
Solution ID = XXXX
or 
Solution = XXXY
The template provided only requests sys_ID and email 

Please advise if a custom template is needed to fuflil this request
