How to find oob risk questionnaires in instance and how to set our own questionnaire in it can anyone help

Hi @VenkataSreC ,

 

You can find Risk assessments (if related to change request) in below table.

Table : change_risk_asmt

All the risk assessments will be configured in 'Assessment Categories'(asmt_metric_category) table.

here you can define your own questionnaires (Assessment Metrics).

Hi @VenkataSreC ,

A risk questionnaire is a set of questions used to assess risks for things like vendors, projects, or systems. Based on the answers, ServiceNow can calculate a risk score.

Go to the Application Navigator (left-hand search bar) Type and open: "Questionnaires"
(under Risk → Assessments → Questionnaires)
You’ll see a list of questionnaires created by ServiceNow or your team.

Look for names like: "Information Security Risk", "Vendor Risk Assessment", and "Third Party Risk", etc. These are the OOB (Out-of-the-Box) ones.

Create Your Own Risk Questionnaire: Risk → Assessments → Questionnaires. Click "New"

Fill in:
A name (like “My Custom Risk Assessment”)
Choose what it’s for (like Vendor, Risk Register, etc.)
Choose Manual for now.
After saving, scroll down and:
Add Sections (like "Security", "Compliance", etc.)
In each Section, add Questions (Yes/No, Multiple Choice, etc.)

Use Your Questionnaire
Go to: Risk → Assessments → Templates
Create a template and link your new questionnaire to it.
Now, whenever you want to assess a risk:
You can use that template.
It will show your custom questions.
The answers will help calculate a risk score.
