Is it with one or all records? Also please add the Year in date just be sure. Try to create a new record and check.

please try to make your date-format in User profile as System (yyyy-MM-dd) and then check what value it shows for those fields.
How are these records created in the table? Is it via some record producer or via some script or via a user clicking New button?

Good morning and thank you for your responses.
At this point the only question I have is how could the Created date/time be later than the Updated date/time? The other fields show up as populated when in list view on the table, although they are blank in the history list view on a record. I can ignore the history list view as long as I can see the values in the list view on the table, but I just don't understand how the Created date/time be later than the Updated date/time.

Tough to answer on this, as maybe some system glitch or someone update or run any script on created date or these records get overwritten by any job or.. so.. Truly either you need to dig out the logs and check.
