Providing end-user with a template on service portal

Hello everyone!

I need some help. We were asked to provide the end-user with a template (excel) they could download, fill in and give back filled in an attached field.

Let's use the green as a Variable. The point was to have a variable here, where the end user could download the file from. Is it possible?
