Help to modify the catalog item

Hi would like to have some ideas to modify the catalog item as below. I need something which is current and easy to understand

Hi @AnisDevG ,
Right now the catalog item has too much instructional text, so users have to stop and read before they even know what to do. The easiest improvement is to simplify and guide, rather than explain everything up front.
A few practical ideas that usually work well:

Rename it to something clearer like
“Request a Service Catalog Change” -
Clearly ask one simple question at the top:
“What do you want to do?”-
Create a new catalog item

Modify an existing catalog item
-
Disable a catalog item

Instead of asking users to highlight text in different colors (NEW / MODIFY / REMOVE), use a mandatory dropdown for the request type. This removes confusion, avoids mistakes, and makes approvals and reporting much cleaner.
You can then move most of the instructions into:

Field help text
-
Short descriptions shown only when needed
For example:

If “Modify” is selected, show a short note like “Please describe what is changing.”

If “Disable” is selected, ask for the catalog item name and reason.
Also, make the template link more obvious (for example, “Download Service Catalog Request Template”) so users don’t miss it.
Overall, the goal is:

Less text on the page
-
Clear choices
-
No color-coding rules to remember
That alone will make the form much easier to understand and use.
