How to find/edit this form

I am wondering how to find the table/form name to be able to edit the fields. I am wanting to add and remove fields.

a)It seems to be a record producer . Can you

Navigate to Maintain Item -> search if any Record producer " Create new contract" is there which creating record in ast_contract

b) if not , check Create a contract
