why have you created a defined related list when there is OOB change tasks related list?
I already informed you can use OOB related list and edit the default filter
Did you try that and it should work fine?

Hi Ankur,
As per requirement i need to create a Related list on the change form with the name
CI Outcomes,
under this CI outcomes Related List , all the change tasks related to that particular change record with the the type CI outcome should be placed.
tasks records are coming , please see the red color highlighted things , i need to implement that under related list > i want Change request CHG0030126 instead of just change Tasks
can you please guide me , how to achieve that

since it's a defined related list you cannot open in new window with the pre-defined filter
That's how OOB defined related list work.
