How to add help text for drop down options?

Hi,

I need to add help text  for one option in drop downlist.  please find the below attached. for "JIRA Canada" need to add help text and what's the process? How to do that please help me on that.

incident record created --> survey triggered---> survey done---> 

i need the string_value of that particular incident to be displayed in the description in  that particular incident 

i have tried by business rules bt not displaying in description field

 how can we do this?
