How to implement pop-up survey to CSP?

Hi, 

We are implementing consumer service portal. We would like to implement a portal usage survey to consumers which pops up after consumer submits form. We have configured that survey triggers when case is created and user is redirected to CSP home page.

We would like that survey pops up as modal to consumer. How we can implement this?

Here is raw example what we would like that survey to look like in portal.
